Chronic Care Staffing, LLC (“CCS”) was founded in 2015, the same year Medicare’s Chronic Care Management CPT 99490 program began. Our headquarters are in Charleston, SC. Chronic Care Staffing has compiled an array of clients ranging from small practices to large health systems. Our clinical team has developed industry-leading documentation, population health initiatives, and Value-Based Reimbursement (VBR) calling protocols to enhance future reimbursement. Active LPN license (CALIFORNIA) Live in a state that falls within MST or PST Experience in geriatrics or primary care Ability to sit and talk on the phone for long periods of time System: We require you to have an i5 or newer Windows processor and to have Windows Pro (not Home). This can be a laptop or desktop. Must live/work full-time in the US
Provide monthly calls and support to patients enrolled in the CCM program. Utilize your nursing skills to educate patients and their families. Monitor patients progress and work with the physicians to ensure quality patient care.
