Remote Chronic Care Management Nurse (LPN) - 1099 Contract Position

The Remote CCM Nurse will play a vital role in coordinating and managing care for patients with chronic conditions, ensuring their health needs are met and promoting their overall well-being. This position is fully remote. The ideal candidate should have a passion for delivering top-tier care to patients in a virtual environment. This is a 1099 contract position.

Manage a caseload of patients with chronic conditions, providing care coordination and support. Conduct virtual patient assessments, develop care plans, and follow up on patients’ health progress. Provide health education and counseling to patients and their families to help manage chronic conditions. Collaborate with interdisciplinary healthcare teams, including physicians, specialists, and social workers, to ensure comprehensive care. Monitor patients’ health data, track progress, and adjust care plans as needed. Utilize telehealth technology to conduct patient check-ins and follow-up consultations. Ensure accurate and timely documentation of patient interactions, assessments, and care plans. Maintain up-to-date knowledge of evidence-based best practices for chronic care management.
