As a Certified Family Nurse Practitioner, you will play a crucial role in delivering comprehensive healthcare, 5 days a week, to patients in a recovery and detox Center. Your responsibilities will include conducting thorough assessments, diagnosing and treating common conditions, and collaborating with healthcare teams to ensure optimal care. Job Responsibilities: Patient Assessment and Diagnosis: • Conduct comprehensive H & P's health assessments on patients. • Formulate accurate diagnoses based on clinical findings and collaborate with healthcare teams for further evaluation if needed. Treatment Planning and Implementation: • Develop and implement individualized treatment plans for pediatric patients, considering their unique healthcare needs and developmental stages. • Prescribe medications, therapies, and interventions based on evidence-based practice guidelines. • Monitor growth and development milestones, and offer guidance on nutrition, safety, and overall well-being. Health Promotion and Education: • Provide health education to patients and caregivers on topics such as growth and development, nutrition, immunizations, and safety. • Promote a holistic approach to healthcare that emphasizes preventive measures. Collaboration and Communication: • Collaborate with doctors, nurses, social workers, and other healthcare professionals to ensure coordinated and comprehensive care. • Maintain open communication with parents or guardians, involving them in the decision-making process. Record Keeping and Documentation: • Maintain accurate and up-to-date medical records, including patient histories, examination findings, and treatment plans. • Adhere to legal and ethical standards regarding patient confidentiality and record-keeping.
