We are seeking a licensed practical nurse (LPN) to join our remote care team. In this role, you will provide ongoing support, care coordination, and patient education to chronically ill patients via phone and digital health platforms. Conduct monthly telephonic check-ins with patients to assess health status, medication adherence, and overall well-being Review RPM data such as blood pressure and glucose levels and escalate concerns to providers Provide patient education on disease management, medication, and lifestyle changes Collaborate with physicians, nurses, and care teams to develop and adjust care plans Document all interactions in the EHR system and ensure compliance with Medicare CCM and RPM guidelines Assist with care coordination, including scheduling follow-ups and referrals as needed
