The Role Schedule: Full-time, Monday to Friday, 40 hours per week Hours: 8:00am to 5:00pm ET Pay: $22.00–$25.00/hour, depending on experience Status: 1099 contractor initially Future pathway: Strong performers may be considered for W-2 employment after 3 months, subject to performance, productivity, compliance standards, and business needs. ColigoMed is hiring experienced Remote LPN/LVN Care Coordinators to support Chronic Care Management and Remote Patient Monitoring programs across the United States. This role requires: • An active compact multistate LPN/LVN license • Direct CCM/RPM or remote chronic care management experience • Chronic disease management experience • Prior remote healthcare work experience • Strong documentation and patient communication skills • Prior experience managing an assigned patient panel or active caseload in a structured remote care environment • Successful management of an active remote care caseload of approximately 150+ patients is strongly preferred This is a patient-facing remote nursing role. You will manage an assigned panel of patients, conduct regular outbound patient calls, document care activity accurately, support care plans, and escalate clinical concerns when needed. This role is best suited to nurses who enjoy structured remote workflows, ongoing patient relationships, chronic disease management, and consistent patient engagement. Why This Role May Be a Strong Fit • Remote nursing role • Monday to Friday schedule • No bedside shifts • Ongoing patient relationships • Structured CCM/RPM workflows • Supportive clinical operations team • Structured onboarding and workflow training provided • Opportunity to be part of a growing digital health company Key Responsibilities As a Remote LPN/LVN Chronic Care Coordinator, you will: • Manage an assigned panel of CCM/RPM patients, including regular outbound calls, follow-ups, care plan support, and patient education. • Support patient onboarding, program explanation, consent documentation, RPM setup, and ongoing program participation for Medicare and Medicare Advantage patients. • Review patient needs, symptoms, medication adherence, care plan progress, barriers to care, and RPM/device issues in accordance with approved workflows. • Document all patient interactions accurately and on time, including call content, duration, follow-up needs, care plan updates, barriers to care, and escalation actions where applicable. • Accurately record actual patient engagement, care coordination activity, follow-up work, and billable-support time in accordance with ColigoMed workflows and CCM/RPM documentation requirements. • Complete assigned patient follow-up tasks promptly and ensure unresolved patient needs, symptoms, medication concerns, device issues, or escalation items are followed through to completion. • Escalate clinical concerns appropriately and promptly to providers and/or the Clinical Operations team. • Maintain care plans in coordination with providers and the Clinical Operations team. • Meet expected productivity, quality, documentation, compliance, follow-up, and patient engagement standards. • Participate in QA review, internal audit feedback, coaching, and process improvement when documentation, follow-up, timekeeping, or workflow issues are identified. Productivity and Quality Expectations This is a structured remote care role with clear expectations for patient engagement, documentation, quality, compliance, and productivity. Successful nurses will be expected to manage regular patient outreach, maintain accurate same-day documentation, follow standardized CCM/RPM workflows, and meet agreed performance expectations once fully trained and when patient volume supports this level. Productivity credit must be supported by accurate documentation, appropriate time records, completed patient follow-up, and compliance with approved workflows. We assess performance across patient engagement, care continuity, documentation quality, compliance, follow-up completion, call quality, escalation judgment, and productivity. We do not assess performance on volume alone. Quality, compliance, and patient care standards are essential. Required Qualifications and Experience Candidates must have: • Active LPN/LVN compact multistate license. • Minimum 6–10 years of nursing experience, including chronic disease management. • At least 2 years of direct experience working in a structured remote CCM, RPM, chronic care management, telehealth, or virtual care company. • Prior experience working in a structured remote care company, CCM/RPM provider, telehealth company, population health organization, or similar service model where nurses use a digital care-management platform to manage assigned patient panels, conduct scheduled outreach, document interactions, update care plans, and support billing/compliance workflows. • Demonstrated experience using a remote care platform, EMR, telehealth system, RPM dashboard, or digital care-management tool as part of daily clinical workflow. • Experience managing an assigned panel of patients in a remote or virtual care environment, including regular outbound patient calls, follow-up tracking, care plan updates, and escalation of clinical concerns. • Successful management of an active remote care caseload of approximately 150+ patients is strongly preferred. • Experience supporting CCM/RPM workflows for Medicare or Medicare Advantage patients, including documentation standards, time tracking, care coordination, and patient engagement requirements. • Strong experience with chronic disease management, including conditions such as diabetes, hypertension, COPD, CHF, obesity, kidney disease, or other long-term conditions. Candidates whose experience is limited to bedside nursing, clinic-based nursing, utilization review, prior authorization, chart review, triage-only work, or general call-center work are unlikely to meet the requirements for this role unless they also have direct structured remote care, CCM/RPM, telehealth, population health, or similar digital care-management experience. Knowledge and Skills • Strong clinical knowledge in chronic disease management and remote patient care coordination. • Proven ability to work independently in a structured remote environment while following standardized workflows, productivity expectations, QA standards, and escalation protocols. • High attention to detail, with strong documentation accuracy and ability to work within clinical, operational, and compliance requirements. • Strong organizational and time management skills, with the ability to manage multiple patients, priorities, and follow-up tasks concurrently. • Excellent communication skills with patients, providers, and internal team members. • Team-oriented, collaborative, and responsive to coaching, feedback, and process improvement. • Bilingual Spanish skills preferred. Remote Work Requirement This position is remote/work from home. Candidates must have a reliable internet connection and a quiet, private workspace suitable for confidential patient communication. Services Schedule This is a full-time remote contractor role, generally up to 40 hours per week, Monday to Friday. Core service hours are generally 8:00am to 5:00pm ET, including an unpaid meal break. Because patient availability may vary, nurses may have the opportunity to support adjusted daytime schedules, including earlier morning or early evening coverage, where needed and agreed in advance. Compensation: This is initially a 1099 contractor role paid from $22.00 to $25.00 per hour, depending on prior remote care experience, skills, licensure, location, direct CCM/RPM background and demonstrated ability to manage high-volume patient engagement.
