Seeking a motivated and detail-oriented LVN for Chronic Care Management, the full-time remote position will manage high-volume outbound patient calls, conduct comprehensive assessments, and develop care plans while utilizing AI tools to enhance clinical documentation and care coordination. Key Responsibilities • Operate in a high-volume clinical call center, managing inbound and outbound patient interactions • Conduct outbound calls to check on patients, addressing health concerns and ensuring timely outreach • Document all patient interactions accurately in real time within the clinical platform Required Qualifications • Active LPN/LVN Compact License required • Passionate about patient care with the ability to relate with empathy and compassion • Proficient with computers, EMRs, and telehealth tools • Strong communication and organizational skills • Experience in a call center or high-volume phone-based work environment preferred
