Full-Time | Flexible Hours | US-Based Only | Fully remote/work from home Lara Health is seeking a compassionate and experienced Medical Assistant (CMA/RMA) or Licensed Practical Nurse (LPN/LVN) to join our virtual care team. This is a fully remote role with flexible scheduling, ideal for someone who enjoys building strong patient relationships, providing ongoing support, and helping patients stay engaged in their care between office visits. This role is centered around monthly patient engagement, virtual check-ins, and supporting patients with chronic conditions through consistent, meaningful communication. Position Highlights Schedule: Minimum 35 hours/week Location: Fully remote (U.S.-based only) Experience Required: Minimum 5 years caring for patients with chronic conditions such as diabetes, hypertension, cardiology, or primary care Role Responsibilities • Conduct monthly virtual patient outreach and check-ins • Build trusting, supportive relationships with patients over time • Provide patients with ongoing encouragement, education, and non-emergency support • Help patients stay engaged with their care plans and health goals • Discuss symptoms, medication adherence, home readings, and general health concerns as appropriate • Support patients in scheduling provider visits and following recommended care plans • Escalate clinical concerns to the appropriate provider or supervisor when needed • Maintain accurate and timely documentation in EMR and internal systems • Collaborate with providers and internal care teams to ensure continuity of care • Perform other duties as assigned Required Skills & Qualifications • Licensure: Active Medical Assistant certification or LPN/LVN license required • Experience: 5+ years in a clinical setting, preferably with chronic care or longitudinal patient support • Language: Bilingual English/Spanish strongly preferred • Excellent communication skills with a warm, patient-centered approach • Strong ability to engage patients consistently and build rapport over time • Strong organizational and time management skills • Ability to work independently and remotely • Proficiency with computers, EMR systems, and virtual communication tools • Must have own device and reliable internet connection • Familiarity with medical terminology and patient education • Passion for working with older adults and delivering preventive, relationship-based care Preferred Experience • Experience with Chronic Care Management (CCM), Remote Patient Monitoring (RPM), or similar virtual care programs • Experience supporting patients between office visits • Comfort managing recurring monthly patient interactions at scale • Familiarity with remote care coordination workflows and EMR-based task management Please Note • Do not apply if you do not meet the required qualifications • This is a contract position and does not include benefits • No agencies or recruiters • Online applications only - please do not call or email • Only shortlisted candidates will be contacted
