Complete intakes and orientations for new clients Build and sustain relationships with all clients, both past and present Review logbooks at the start of each shift Ensure that all client follow-ups are conducted as required Provide assessments, referrals, and advocacy on behalf of clients Coordinate services with social services, mental health, etc. Establish individualized interventions and treatment plans Collaborate with internal staff to monitor medication Record and maintain documentation in case records Participate in case conferences and staff training Conduct home visits when necessary
