THIS ROLE IS REMOTE. MUST HAVE AN RN COMPACT LICENSE FOR MULTIPLE STATES TO BE CONSIDERED FOR THIS ROLE. Monday through Friday 8:00 AM - 5:00 PM CST Pay Rate is $34.00 per hour

Requirements

An active and unrestricted compact RN License Case Management Certification CCM preferred 2 years of direct clinical experience, e.g., hospital setting or alternative care setting such as ambulatory care or outpatient clinic/facility. 2 years Healthcare and/or managed care industry experience. Proficiency in Microsoft Office (Word, Outlook, and Excel)

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.
