Remote Nurse LPN (Part-Time 24-32hrs/wk)

Founded in 2018, we are a home-based medical practice dedicated to providing exceptional primary care to seniors in the comfort of their own homes, assisted living facilities, and group homes throughout Middle Tennessee.

We are looking for a compassionate, self-directed LPN who is passionate about geriatric care and wants to move away from the "conveyor belt" feel of traditional clinics to make a real impact on patient lives.

As an LPN with NP Housecalls, you will play a vital role in our care team. You will support our Nurse Practitioners in delivering high-quality medical services, managing chronic conditions, and ensuring our senior patients receive the personalized attention they deserve. This is a highly autonomous role that requires a work at home environment safe to speak to patients over the phone and access to a strong internet connection. Care Coordination: Act as a bridge between the patient, their family, and the NP. Help manage referrals, pharmacy communications, labs and durable medical equipment (DME) orders. Documentation: Tech knowledgable to maintain accurate and timely electronic health records (EHR) for all patient encounters. Patient Education: Empower patients and their caregivers by providing education on medication management, chronic disease self-care, and preventative health. Triage: Assist with phone triage and responding to patient/family inquiries regarding symptoms or care plans.
