We are seeking a self-motivated, energetic, detail oriented, highly organized, tech-savvy Licensed Practical Nurse (LPN), Registered Nurse (RN), or Licensed Social Worker (LSW) to serve as a PASSPORT Case Manager/MyCare Level of Care Assessor.

• Active Ohio or multi-state RN, LSW, or LPN license Strong organizational and communication skills Ability to work independently in a remote environment Reliable transportation, valid driver’s license, and auto insurance
