Manage incoming vital RPM alerts with precision Follow SOP & escalation pathways for incoming alerts Provide accurate, valid & complete information using appropriate methods/tools Respond to alerts within the necessary timeline Document incidents accurately Demonstrate excellent communication skills, adhering to customer service expectations in every call interaction Make outbound phone calls to patients on Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs Monitor uploads and readings, ensuring compliance with care plan parameters Go the extra mile; we value hardworking, engaged, honest, and reliable team members Make outbound followup calls to post-acute patients during RPM alerts
