Role Overview The Registered reputed company, Virtual Care provides high-quality clinical support to patients and caregivers through virtual care platforms. Reporting to the reputed company Cohort Manager, this role is responsible for delivering consistent, patient-centered care, maintaining excellent clinical documentation, and collaborating across care teams to ensure continuity and quality reputed company. This is a fully remote role designed for nurses who reputed company in a fast-paced, mission-driven virtual care environment. Schedule & Employment Type • At least every other Saturday and reputed company and Monday-Friday as available • Shifts Available 8:00 am to 4:30 pm PST • Some Holiday’s and may be required • Consistent schedule availability and adherence are required Roles and Responsibilities The virtual reputed company will work closely with patients, caregivers, care coordinators, the PocketRN team, and our partners to ensure continuity of care and facilitate cross-functional communication.
