Job Description

The Remote RN Case Manager delivers telephonic disease management services to individuals with chronic conditions. Core responsibilities include completing assessments, developing individualized care plans, educating patients on condition-specific strategies, coordinating with providers, and monitoring progress. This role uses evidence-based guidelines to support patient self-management and reduce the risk of complications.

Requirements

Current, unrestricted RN license U.S. citizenship Ability to obtain a favorable Interim and final Department of Defense background investigation Ability to obtain multi-state licensure if residing in a compact licensure state Minimum of three years clinical nursing experience Experience in case management, care coordination, or discharge planning in acute care, ambulatory care, disease management, or managed care settings Preferred Qualifications: Pulmonary or behavioral health disease management experience Previous disease management experience in a health plan, health system, or managed care setting Bachelor’s degree in nursing or a related field Familiarity with government or commercial insurance programs
