RN Care Coach -Chronic Care (Remote)

Active RN license in your state of residence (multi-state compact license preferred) 2–4+ years of experience in managed care or case management Case Management Certification (CCM) preferred Strong communication and organizational skills Proficiency in Excel, Word, Outlook, and other Microsoft 365 tools Ability to multitask and adapt in a fast-paced, evolving environment Preferred Skills & Qualifications: Bilingual in English and Spanish highly required – many of the patients we serve are Spanish-speaking, and this skill significantly enhances the level of care and connection we provide Experience with motivational interviewing techniques A patient-first, solution-oriented mindset Knowledge, Skills and Competencies: Demonstrated ability to adapt and be flexible in an environment that moves at a rapid pace, where change is common and frequent. Solid working knowledge of Excel with proficient skills in MS Word, PowerPoint, and Outlook Ability to manage multiple projects and conflicting priorities. Excellent time management skills. Strong Problem solving and critical thinking skills. Excellent communication skills; verbal and written, and organizational skills Education and Experience: Active RN license in state of employment with a multi-state compact license for States served.
