Job Description

Location: In-home visit locations vary, most work is performed remotely. Position Type: Mostly flex time with in-home visits for new clients. Are you a Registered Nurse with a passion for ensuring high-quality care for senior clients in their homes? We are seeking a dedicated and compassionate Registered Nurse Care Coordinator to be the primary point of contact for our clients and their families, ensuring they receive optimal care. In this role, you will coordinate care plans, manage communication with clients and staff, and oversee the delivery of care to ensure it meets our high standards. As we are just getting started this role will start off part-time and grow as the agency grows.

Responsibilities

Serve as the primary point of contact for clients and their families via multiple communication channels and addressing any questions, concerns, or requests. Work closely with the scheduling department to modify and maintain the client's care schedule, making adjustments as needed. Coordinate comprehensive care for each client among all team members involved in their care. Maintain accurate documentation of all client and caregiver interactions. Conduct timely 90-day care plan reviews and nursing reassessments. Supervise CNA staff and ensure they provide exceptional care. Observe, assess, and document patient symptoms and progress, notifying the HIPAA representative if there are any changes in the patient’s condition. Ensure the completion of all patient care paperwork in an efficient and timely manner. Monitor infection control practices in all areas and ensure compliance with policies. Participate in quality improvement initiatives and contribute to the development of CNA/GNA orientation and training programs.
