Inhouse Physicians
We are seeking a highly skilled and motivated RN Care Navigator to join our IHP team. You will be an integral part of our population health program aimed at revolutionizing our approach to care coordination and delivering exceptional outcomes for our client members. As the program’s Care Navigator, you will play a pivotal role in identifying and closing gaps in care for high-risk members of our corporate clients, making a tangible impact on their health and well-being. Reporting to the Associate Director of Worksite Health, you will work closely with our executive leadership team (ELT), cross-functional partners, and other team members to ensure the success of this high impact program.
Job Skills/Qualifications: 2-5 years of experience in care navigation/coordination Proficient in care coordination and navigation processes Familiarity with navigating benefit plans and population health/worksite health programs Knowledge of ICD-10 codes and experience with claims data Excellent organizational and communication skills, both written and verbal Values Fit: Accountability Collaboration Integrity Innovation Software/Systems: Experience with population health data analytics platforms Preferred Licenses/Certifications/Education: Health coach or wellness coach certification Registered Nurse Bachelor's degree preferred Compensation: Salary with performance bonus potential.
Identification of High-Risk Members and Associated Gaps in Care Utilize population health data analytics platforms to identify high risk members Conduct assessments to identify specific gaps in care for each high-risk member Collaborate with other team members and departments to create tailored care plans for individuals High-Risk Member Outreach & Education: Initiate proactive outreach to high-risk members to engage them in their care Provide empathetic and compassionate support to address their concerns and needs Educate members on general clinical education, available health resources, and benefit plans Host health education sessions for eligible members Assist with generating member education materials Coordination/Scheduling of Care for High-Risk Members: Work closely with healthcare providers and facilities to coordinate and schedule necessary care services Monitor the progress of care plans and ensure timely follow-ups Advocate for members to ensure they receive the best possible care and treatment Ad hoc duties as assigned
