The Community Health Worker will work from home and telephone at-risk patients to check-in on any symptoms, determine unmet needs, and coordinate proper escalations with clinicians or community resources. Excellent communication skills: both verbal and in writing. Ability to communicate with older adults, varied socioeconomic backgrounds, and educational levels in an understanding, non-judgmental manner is imperative. Dependability. Timely and complete documentation to ensure any issues are properly handled per company standards. Good time management skills and ability to prioritize tasks. Ability to work both independently and collaboratively. Excellent customer service skills – ability to understand and exceed customer expectations while demonstrating the highest standards of care, respect, and confidentiality. Experience in a healthcare setting providing care coordination and coaching of geriatric patients is required since that is the patient profile KinergyCare supports. Must have a computer and reliable Broadband internet service.
