Job Title: Registered Nurse/Health Coach Company: Synergy Healthcare USA Location: Remote Job Overview: Synergy Healthcare is seeking a dedicated and compassionate, full-time Registered Nurse (RN) Health Coach to join our growing team. Working exclusively with eligible employees and dependents of one of our largest NC-based clients, the RN Health Coach will play a pivotal role in promoting patient wellness, managing chronic conditions, and enhancing overall health outcomes through personalized coaching and education. This position requires a blend of clinical expertise, strong communication skills, and a passion for helping others achieve their health goals.
Current Registered Nurse (RN) license in North Carolina. Bachelorās degree in Nursing (BSN) and Certified Health Coach (CHC) is preferred. Minimum of 3 years of clinical nursing experience; experience in health coaching or wellness programs is a plus. Strong knowledge of chronic disease management, preventive care, and lifestyle medicine. Excellent communication, organizational, time management, follow up, and interpersonal skills, with the ability to establish trust and rapport with diverse patient populations. Proficient in using electronic health records (EHR) and health coaching platforms. Ability to work independently and as part of a team in a fast-paced environment.
Conduct comprehensive assessments to evaluate patients' health status, lifestyle, and health goals. Develop individualized health coaching plans in collaboration with patients, focusing on behavior change, chronic disease management, and overall wellness. Educate patients on disease prevention, nutrition, exercise, and self-management techniques. Provide ongoing support and motivation to patients, fostering accountability and engagement in their health journey. Collaborate with multidisciplinary teams, including Synergy Nurse Advocate, physicians, dietitians, and therapists, to ensure comprehensive patient care. Monitor patient progress and adjust health coaching plans as necessary to meet evolving health needs. Utilize evidence-based practices to inform coaching strategies and interventions. Maintain accurate and up-to-date documentation of patient interactions and progress in compliance with healthcare regulations. Participate in continuous professional development and stay current with best practices in health coaching and nursing.
