RN, Medication Therapy Management (Predominately Remote)

The Medication Therapy Management (MTM) RN is responsible for medication reconciliation. This position is responsible for identifying patients needed for medication reconciliation and for preparing lists requiring pharmacist consultation.

Review health plan and discharge data to identify patient population requiring medication reconciliation Ability to effectively review medication lists and complete reconciliation function Outreach to patients to review medications Outreach to physicians for any discrepancy on medication lists that need to be addressed Prepare medication lists for patients requiring a pharmacist consultation Successfully learn and navigate various computer programs And Epic/Nextgen EMRs. Report performance data as requested Communicate clearly and thoroughly, provide excellent customer service and be able to work both independently and as part of a team. Other related duties and projects, as assigned.
