Remote Patient Monitoring (RPM) Passive Device Center serves our patient population in the navigation, prevention, and management of their health through our RPM program to help prevent hospitalizations. The RN Team Lead of RPM serves as the primary point of contact for inquiries regarding the daily operations of the Passive Device Center RNs and ensures that the Clinical Manager RPM and Director of RPM are kept informed. This role will also be responsible for communicating effectively with both team members and management to achieve project objectives. They will assist in the onboarding of new team members and provide training. They will communicate and collaborate with management of all alerts received via remote technological devices leveraging dashboards/reports, assuring integrity and appropriateness of reporting elements, and clinical triaging of the findings per protocol established. The RN Team lead RPM will also assist the RN Clinical Manager with audits of staff members, attending meetings, as needed, encouraging teamwork, delegating tasks appropriately, and assisting in the daily assignment.

Requirements

Registered Nurse license adheres to Scope of Practice High level of professionalism Excellent communication skills to interact with team members and facilities. Utilizes Critical Thinking. Knowledge and skill in chronic disease and remote patient monitoring management or equivalent education/experience Experience in managing/reviewing monitors related to alerts and accessing and reviewing resident EHR for related information to health condition PREFERRED SKILLS: Strong verbal/written communication and presentation skills Experience in working with team members to promote ideas, issues, and initiatives within a constructive group framework Knowledge of principles and processes for providing customer services, which includes evaluation of customer satisfaction. Ability to organize and prioritize tasks Ability to work independently, be attentive to detail and maintain a positive attitude.

Responsibilities

Serve as initial point of contact for questions and serve as a subject matter expert to the team. Assist with onboarding training of new team members. Ensure protocols related to alerts are adhered to as established. Responsible for maintaining adherence of metrics of volume, type, protocol adhered to, and timeliness of notification to facilities in efficient productive manner. Monitors vital signs and alerts in assigned facilities, the escalation of alerts according to appropriate algorithm, and the contact made with facilities when necessary. Communicates with Clinical Manager of RPM, and Director of RPM alert center regarding quality, system capacity, and resource requirements, facility and/or provider feedback. Meets established standards for productivity. Develops relationships across facilities organization and within TapestryHealth to facilitate communication and improvement of program. Establish a meaningful rapport that builds trust, open communication, and motivation to make a positive change in the patient's health. Ensure the team adheres to protocol established with the facilities established in an outlined workflow process. Collaborate with care management team members to work in partnership and improve workflow process. Attend meetings as designated by RN Clinical Manager, Director of RPM and/ or Senior Vice President of Care Management. Performs all duties, responsibilities, and adheres to the Nurse Practice Act. Ensures timelines are met. Protects patient information and adheres to standard of confidentiality with all patient information. Contributes and participates in any performance improvement and continuous quality improvement activities within the Care Management work realm. All other duties as assigned.
