To support a growing population health initiative, the part-time Remote Complex Care Nurse Practitioner (SC licensed) will provide telephonic and video-based follow-up care for patients post-hospital discharge, focusing on medication reconciliation, symptom screening, and care coordination across various settings. Key responsibilities Cover care needs for the Transitions of Care program through telephonic outreach and scheduled visits Conduct medication reconciliation, symptom screenings, and order necessary follow-up services Collaborate with nursing teams and maintain accountability for individual performance targets while contributing to team goals Required qualifications Certified Nurse Practitioner in Adult, Family, or Geriatrics with active SC licensure 5+ years of post-graduate clinical experience in Primary Care or Geriatrics Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions Comfort in a digital-first environment with experience in virtual care delivery Bilingual in English and Spanish is a strong bonus
