Conduct comprehensive virtual consultations for patients in post-acute care facilities across Florida. Diagnose, treat, and manage a wide spectrum of hematology and oncology conditions, including anemia, clotting disorders, blood cancers, and complications related to chemotherapy or radiation therapy. Develop and adjust evidence-based treatment plans, including prescribing medications, managing transfusions, and monitoring chemotherapy regimens. Order and interpret diagnostic tests, including complete blood counts (CBCs), coagulation studies, and imaging, to guide treatment decisions. Collaborate closely with on-site nurses, facility medical directors, and primary care providers to ensure seamless transitions of care and effective hematology/oncology management. Provide clinical leadership and guidance for hematology and oncology care programs within partner facilities, promoting best practices and optimizing patient outcomes. Ensure meticulous and timely documentation of all patient encounters within the EMR/EHR system in compliance with all relevant Florida and federal regulations. Educate facility staff on best practices for hematology and oncology care, including treatment adherence, medication management, and patient education.
