Active Florida Nurse Practitioner License: Must hold a current, unrestricted NP license in the State of Florida (a dealbreaker!). Education: Masterās or Doctorate degree in Nursing (MSN/DNP) with a specialization in Pulmonology or relevant experience in respiratory care. Experience: Minimum of 2 years of clinical experience in pulmonology. Previous experience in post-acute care, skilled nursing facilities, or long-term care environments is highly preferred. Liability Insurance: coverage is provided by the company to nurse practitioners. Technical Skills: Proficiency in EMR navigation (experience with common SNF EMR systems is a plus) and comfort utilizing telemedicine technology. Skills: Strong commitment to quality improvement, excellent communication skills for collaborating with diverse facility staff, ability to work autonomously, and clinical empathy tailored to the geriatric and post-acute population.
Conduct comprehensive virtual consultations for patients in post-acute care facilities across Florida. Diagnose, treat, and manage a wide spectrum of pulmonary conditions, including chronic obstructive pulmonary disease (COPD), asthma, interstitial lung disease, pulmonary fibrosis, and sleep apnea. Develop and adjust evidence-based treatment plans, including prescribing medications, oxygen therapy, and other therapeutic interventions. Review patient records, recent hospital discharge summaries, laboratory data, and imaging studies to formulate comprehensive, facility-appropriate treatment plans. Collaborate closely with on-site nursing staff, facility medical directors, and primary care providers to ensure seamless transitions of care and effective pulmonary management. Provide clinical leadership and guidance for respiratory care programs within partner facilities, promoting best practices and optimizing patient outcomes prevention strategies, treatment adherence, and patient education.
