Job Description: • Virtually evaluate patients entering the program and assist in creating care plans based on patient-specific needs and predesignated protocols • Respond to alerts escalated from the nursing team regarding deviations in vitals, physiologic data, symptoms, and weight changes • Independently manage clinical-status changes within the APP’s scope of practice • Escalate out-of-scope clinical changes to the lead MD • Liaise with the nursing team to ensure medications, labs, and virtual appointments are ordered and communicated • Partner with the nursing team to reconcile medications on program entry and exit • Serve as the point of contact for patients’ primary care teams and relay tailored information • Educate patients about healthy behaviors and self-management • Manage a growing patient panel and contribute to development of a telemedicine program using medication-management titration algorithms and patient-management principles Requirements: • 2+ years as a Board-Certified Nurse Practitioner specializing in heart failure and/or internal medicine in an ambulatory or inpatient setting • Actively licensed in California • Willingness to obtain additional state licenses as needed • Fluency in English and Spanish is highly preferred • Previous experience overseeing complex chronic care programs strongly preferred • Previous telehealth experience strongly preferred • Willingness to work in a changing, dynamic environment with a growing company • Ability to work from home in a HIPAA-compliant environment is required • Motivation to provide health education and grounding in health promotion techniques • Ability to learn and proficiently use multiple EMRs and various communication applications, including Google products • Full-time availability Monday through Friday, 8:00 AM to 4:30 PM PST Benefits: • Company-sponsored health, dental, and vision insurance • 401K program • PTO program including eight paid company holidays
