Welcome To General Medicine – The Post-Hospitalist Company We are a collaborative practice of board-certified Physicians and Nurse Practitioners who specialize in the care of patients residing in post-acute, long-term care and assisted living facilities. Our clinicians work exclusively in these type of facilities rather than trying to manage an outpatient office practice and also making hospital and SNF rounds. Also, we do not compete with doctors in an inpatient hospital practice. They are the Hospitalists and we are Post-Hospitalists. Focusing exclusively means no more waiting for return calls or infrequent facility visits that leave your nursing staff considering hospitalization or re-hospitalization when a resident’s condition is compromised or deteriorating. It also means our practitioners are available to make early assessments, take early interventions and monitor their efficacy on site.

In this role, you will actively practice medicine while also providing clinical leadership—helping shape workflows, triage protocols, and standards of care in a fast-growing digital health environment. You’ll work within a sophisticated virtual platform that includes triage, chat and video visits, rapid specialist input, and expedited labs, imaging, and referrals within patients’ insurance networks. This role is ideal for a physician who enjoys both hands-on clinical care and leading clinical systems at scale. Clinical Care & Medical Decision-Making Provide high-quality virtual care across a full spectrum of acuity and complexity Confidently manage triage scenarios and determine appropriate escalation pathways Utilize rapid specialist input, labs, imaging, and referrals to deliver efficient, coordinated care Clinical Leadership & Oversight Support and oversee clinical workflows, policies, and care standards Provide guidance and leadership to other clinicians within the platform Contribute to the development and refinement of triage protocols and clinical best practices Quality, Safety & Efficiency Ensure clinical decisions align with evidence-based medicine and patient safety standards Partner cross-functionally to improve care delivery, efficiency, and patient outcomes Identify gaps in workflows and propose solutions to improve quality and throughput Technology & Platform Adoption Quickly learn and master platform tools, clinical workflows, and operational processes Adapt clinical practice to a virtual-first environment without compromising care quality
