The Optum Serve Physician Assistant or Nurse Practitioner provides clinically competent delivery of quality virtual medical care services required during Optum Serve operations. The PA or NP is responsible for providing clinically competent medical care services, within their scope of practice. The PA or NP working remotely on behalf of Optum Serve is responsible for safe patient care and ensuring adherence to all Optum Serve Policies and Procedures. All PA or NP shall meet the qualifications for their specialty and perform essentially the same functions, within the scope of acceptable standards for their technical professional discipline, as those required by DoD or government service technical professionals of similar experience and in similar duty assignments. This position works in collaboration with other providers as well as other team members of the healthcare team to complete Virtual health assessments in non-traditional healthcare settings. This provider is responsible for virtual patient triage, assessments, and referrals of all presenting patients to Optum Serve Telehealth. Primary Responsibilities: This role is entirely remote and requires working in a non-traditional clinical setting. It is also (virtually) customer-facing with high expectations for operational excellence. The noise level is moderate and there is no risk of exposure to infectious diseases and blood-borne pathogens. Hours may vary based on location, patient volume, and business needs. This list of essential functions is not exhaustive and may be supplemented and changed as necessary. • Excellent critical thinking, problem solving, verbal and written communication skills, in addition to patience and a high-level of customer service to all patients and fellow staff • Always acts professionally with minimal supervision as a representative of Optum Serve • Possesses solid organizational skills and the ability to prioritize, solve moderately complex problems while working both independently and collaboratively with others • Maintains compliance with Optum Serve and Federal, State, and Local regulatory, contractual, and organizational guidelines including HIPAA as outlined in training and policies/procedures, quality assurance and improvement processes • Organizes workload, sets priorities, completes assignments in a timely manner and utilizes resources appropriately while complying with organizational standards • Dedicated to building rapport with team members, internal and external customers, and management team by demonstrating a solid customer service orientation and a continuous positive image of Optum Serve • Is punctual for work attendance and abide by work schedule • Practices within mandated scope of licensure/certification • Provide excellent patient care within the scope of PA or NP credentialed privileges • Analyze and interpret histories, symptoms, physical findings, and diagnostic information • Maintain complete and detailed records of patients' plans of care, documenting care within an appropriate time frame • Conduct virtual medical assessment • Ensure appropriate follow-ups for all referrals • Ensure patient assessments are being carried out as per site guidelines • Manage new medical problems and appropriate follow-up on any ongoing medical problems • Perform other duties as assigned by Leadership • Apply proper patient identification practice and HIPPA compliance • Provide virtual support in emergent situations to patients who have medical needs while waiting for emergency response team • Remain up to date on all contract requirements • This list of essential functions is not exhaustive and may be supplemented and changed as necessary Emergency Response: • Responsible for assessment of patient's emergent needs in a nontraditional setting • Attune assessment and critical thinking skills to ensure appropriate care can be delivered safely and within the scope of capabilities of the available resources • Skilled in emergency response Shift Expectations: • There will be 3 shift options: 8 am CST, 9 am CST or 10 am CST start • Each shift will be 8.5 hours and include two 15 minute breaks and one 30 minute lunch break • All shifts must work one weekend rotation per month with the choice of a weekday off that week Mental Demands: Reading, writing, attention to detail, confidentiality, problem-solving, ad hoc decision-making skills, math skills, reasoning skills, oral communication, written communication, customer contact, multiple concurrent tasks, stress management skills, interpersonal skills, ability to work independently with minimal supervision. Optum Home & Community Care is part of UnitedHealth Group and provides telephonic case management services to help members navigate the health care system and access support services. As an RN Case Manager, you would assess members' health status, identify gaps in treatment plans, establish care goals, coordinate with multidisciplinary teams including mental health clinicians, provide patient education and coaching, make referrals to community resources, and facilitate transitions between care settings—primarily through phone calls in a high-volume customer service environment.
