The role you’ll contribute: The CxC Virtual Care Nurse coordinates care, educates clients, and provides protocol-driven interventions within the scope of practice that have direct influence on clinical and whole health outcomes. It is a dynamic and systemic collaborative approach to providing and coordinating health care services to a defined population across the continuum. Responsibilities include virtual assessment of healthcare needs, collaboration, and coordination with health care providers, and development of plan of care with accurate documentation of these activities. Expectations of this role include providing exceptional customer service, striving to anticipate and meet the needs of AdventHealth consumers, treating all consumers and colleagues with dignity and respect, and working collaboratively with colleagues to achieve quality and performance standards. The nurse reports to department leadership and maintains a working knowledge of AdventHealth service offerings, service workflows, and CxC technologies necessary to perform all job functions.
The expertise and experiences you’ll need to succeed : Effective communicator in English, both orally and in writing. Ability to type at a speed of 45 words per minute, at minimum. Proficiency in Microsoft Suite programs and Adobe. Technological aptitude to master additional programs and technologies, including CRM, telephony, electronic health records, and other potential systems. Associate of Science degree in Nursing Graduate of an accredited school of nursing Minimum (2) years of clinical experience Multistate licensure as a Registered Nurse (RN) Basic Life Support (BLS) certification
Demonstrates excellent customer-care skills. Provides virtual clinical care within their nursing scope of practice. Competent in remote patient monitoring and uses critical thinking skills and decisive judgment to provide triage to symptomatic consumers in accordance with scope of practice. Elicit questions, concerns, and information from caller to assess symptoms, determine acuity, identify appropriate algorithm/protocol, and develop patient care plan and follow-up. Provide professional advice, education, and options based on assessment, established algorithm/protocol, and clinical judgment; when appropriate, escalate interactions to appropriate emergency response services. Provide referrals to healthcare providers and community resources when appropriate; assist with follow-up or appointment scheduling when needed. Utilize proper escalations when issues arise and see them through to resolution. Adhere to all established workflows, scripting, and department greetings. Thoroughly document all interactions and consistently update consumer records in the relevant technology system(s). Maintain knowledge of CxC services and AH offerings to properly direct inquiries when outside of service's scope. Perform other duties, as assigned by the Supervisor or other leaders.
