Virtual Psychiatric Nurse Practitioner - Provide Care to Seniors Across California

We are seeking a highly skilled Virtual Psychiatric Nurse Practitioner to provide comprehensive mental health care to seniors across California. This role involves delivering psychiatric evaluations, medication management, and ongoing support through telehealth platforms, ensuring seniors receive quality mental health services wherever they reside. The ideal candidate will have extensive experience in geriatrics, behavioral health, and telehealth systems, with a strong background in medical documentation and patient assessment. This position offers the opportunity to make a meaningful impact on seniors' lives by delivering compassionate, accessible psychiatric care remotely. As a Virtual Psych NP at Avail Health at this early stage offers a unique opportunity to influence the development of best practices in the deployment of technology-enabled clinicians into locations where vulnerable populations reside, while also providing a vital role in the identification and management of chronic illness. You will work closely with a multidisciplinary team, including Geriatric NPs, nurses, physicians and therapists, as well as collaborate with the clinical operations team to deliver comprehensive chronic illness assessments. Your collaborating physician will be Dr. Jennie Byrne, an esteemed clinical educator, thought leader, Psychiatrist and Neuro-scientist. Seeing Medicare patients remotely across California, you will evaluate and treat Dementias, Geriatric Psychiatric Illnesses and Serious Chronic Mental Illness. Clinicians see no more than 2 patients per hour and all new evals are 60 minutes. Avail’s platform provides rich data on patients and includes asynchronous screenings, ambient AI scribing, AI clinical interpretation and diagnostic/coding support. Many of our patients are also enrolled in therapy and care management services so collaboration and non-visit-based actions are encouraged and reimbursed. Flexibility of scheduling allows our clinicians to fit this role into their existing schedule. Clinicians schedule all of their follow ups so they can manage the schedules better.

Conduct thorough psychiatric evaluations and assessments of senior patients via telehealth platforms, utilizing EMR and EHR systems such as Epic, Cerner, and eClinicalWorks. Develop individualized treatment plans including medication management, psychopharmacology, and therapy adjustments tailored to geriatric patients. Prescribe and monitor psychiatric medications, including those requiring specialized knowledge such as botulinum toxin treatments when applicable. Collaborate with primary care providers, specialists (e.g., cardiology, neurology), and caregivers to coordinate comprehensive care plans aligned with patients’ overall health needs. Manage acute psychiatric issues such as crisis intervention, triage, and urgent care referrals while maintaining compliance with HIPAA regulations. Document all patient interactions accurately in medical records, ensuring adherence to clinical research standards and coding requirements like ICD-10 and CPT coding. Provide education on mental health management, medication adherence, and wellness strategies to patients and their families or caregivers. Stay current with advancements in geriatrics, dementia care (including Alzheimer’s and memory care), hospice & palliative medicine, and infectious disease care relevant to senior populations. Participate in telehealth training sessions to optimize virtual care delivery techniques and ensure compliance with NCQA standards.
